This was most recently documented on or about May 29, 2018 when the employer did not enforce the use of a gas meter to test the atmosphere of a permit required confined space. This was most recently documented on or about May 29, 2018 when the employer allowed employees to enter into a confined space without proper training. First aid responder attended rescue without confined space training. Employees were exposed to an atmosphere with low levels of oxygen.
