29 CFR 1926.652(c)(2)(i): Design of support systems, shield systems, or other protective systems that are drawn from manufacturer's tabulated data were not in accordance with all specifications, recommendations, and limitations issued or made by the manufacturer.     On or about May 31, 2018, an employee working in a 5'10" deep trench to remove loose fill to expose the existing water line was not adequately protected from a cave-in.  A Lee Jensen trench box was not installed, per the tabulated data, exposing an employee to the hazards associated with working in a trench box that was missing one spreader bar.     Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

Performance Construction and Engineering, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.652(a)(1), which was contained in OSHA inspection number 1158614, citation number 1, item number 2 and was affirmed as a final order on September 1, 2016, with respect to a workplace located at 421 Valley Lane Arlington Heights, IL 60004.    Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
