29 CFR 1926.652(c)(2)(i): Design of support systems, shield systems, or other protective systems that are drawn from manufacturer's tabulated data were not in accordance with all specifications, recommendations, and limitations issued or made by the manufacturer:  (a) Jobsite at River Road and Chestnut Ridge Drive, Naperville, IL 60540 - On or about June 18, 2024, the employer failed to ensure that the Efficiency Production, Inc. XLAP-66 Aluminum Shield, serial no. 149036, was assembled and installed in accordance with the manufacturer's instructions. The shield was inside an east west running trench 7.5 feet deep. A lower spreader was not installed on the west end of the shield and the three spreaders that were installed to the sidewalls were missing connecting pins thereby exposing an employee assigned work in the excavation to crushing hazards in the event of a cave-in.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

XLAP-66 Aluminum Shield, serial no. 149036 prior to an employee performing work within thereby exposing the employee to crushing hazards in the event of a cave-in.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
