The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: On or about May 31, 2018 an employee changing out the hydraulic cylinder on the vacuum lift used with the Bystronic BySprint Fiber 3015 laser did not utilize energy control procedures and/or equivalent measures to ensure there was no potential for hazardous energy release.
