29 CFR  1910.28(b)(1)(i): (1) Unprotected sides and edges.  (i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following:    (A) Guardrail systems; (B) Safety net systems; or (C) Personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems.   a) On or before 2/21/2024 the employer did not require the use of fall protection system for employees who were exposed to the fall hazards of approximately 9' while performing such tasks as taking width measurement of the steel after coming out of the slitter. During the measurements process, employees would climb up onto the conveyor system exposing employees to a 9 ft. fall while on the conveyor system for equipment such as but not limited to the; Delta Slitter (model # DSS-111).

29 CFR  1910.147(c)(1):Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  a) On or before 2/21/2024 the employer did not ensure that the employer's energy control program adequately addressed the following topics including energy control procedures, employee training and periodic inspections, in that the current program required employees to fill out a form on lockout versus providing a machine specific procedures for the employees performing servicing and maintenance on equipment such as but not limited to; Delta Slitter line (model # DSS-01) to follow to ensure that through training the authorized employees understood the hazardous energy sources and the procedures to secure such hazardous energy sources; electric and hydraulics.

29 CFR  1910.147(d):Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  (1) Preparation for shutdown. Before an authorized or affected employee turns off a machine or equipment, the authorized employee shall have knowledge of the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy. (2) Machine or equipment shutdown. The machine or equipment shall be turned off or shut down using the procedures established for the machine or equipment. An orderly shutdown must be utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage. (3) Machine or equipment isolation. All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). (4)Lockout or tagout device application. (i)Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees. (ii) Lockout devices, where used, shall be affixed in a manner to that will hold the energy isolating devices in a "safe" or "off" position. (5)(i) Following the application of lockout or tagout devices to energy isolating devices, all potentially hazardous stored or residual energy shall be relieved, disconnected, restrained, and otherwise rendered safe. (ii) If there is a possibility of reaccumulation of stored energy to a hazardous level, verification of isolation shall be continued until the servicing or maintenance is completed, or until the possibility of such accumulation no longer exists. (6) Verification of isolation. Prior to starting work on machines or equipment that have been locked out or tagged out, the authorized employee shall verify that isolation and deenergization of the machine or equipment have been accomplished.  a) On or before 2/21/2024 the employer did not ensure that (d)(1) through (d)(6) were followed by an authorized employee performing servicing and maintenance operations on equipment such as but not limited to; Delta Slitter Tension Stand (Model # DSS-01) . Employees were exposed to unexpected startup when performing changing of the cleaning pads for equipment such as but not limited to the Delta Slitter (model # DSS-01).

29 CFR  1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  a) On or before 2/21/2024, the employer did not ensure that the point of operation where employees were working, for machine such as, but not limited to; the Delta Slitter Tension Stand area cleaning pads (model #DSS-01) area was inadequate due to a lack of guarding method in place that when the current sliding guard is moved out away from the opening while in production mode employees were not exposed to a point of operation caught in hazards.

Electric equipment may not be used unless the following markings have been placed on the equipment:  (i) The manufacturer's name, trademark, or other descriptive marking by which the organization responsible for the product may be identified; and (ii) Other markings giving voltage, current, wattage, or other ratings as necessary.  a) On or before 2/21/20214, the employer did not ensure that electrical equipment such as but not limited to: The electrical disconnect for the Delta Slitter (model # DSS-01) was properly marked indicating voltage and what it supplied power too.
