29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: On or about May 17, 2018, an employee was exposed to a fall hazard while standing on the concrete motor base of the Lysine Main 31 Siemens Motor, Model Number 140, Serial Number 1-5110-65517-04-3, while changing an agitator motor inlet filter without the use of fall protection. The employee was exposed to a fall distance of 4.2 feet.
