29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    a)  Press Department, on or about 6/1/18:   Employees are exposed to potentially injurious infrared light in that they were not wearing the appropriate eye protection while working around furnaces and metal forgings 1900 to 2100 degrees Fahrenheit.      b)  Hammer Department, on or about 6/1/18:  Employees are exposed to potentially injurious infrared light in that they were not wearing the appropriate eye protection while working around furnaces and metal forgings 1900 to 2100 degrees Fahrenheit.      c)  Ring Roll Department, on or about 6/1/18:  Employees are exposed to potentially injurious infrared light in that they were not wearing the appropriate eye protection while working around furnaces and metal forgings 1900 to 2100 degrees Fahrenheit.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a)  Maintenance, on or about 6/1/18:  Edlund multi-drill press was used without a guard to prevent contact with the rotating chucks and tooling.    b)  Maintenance, on or about 6/1/18:  Oster Rapiduction threading machine used to thread 7-inch long bolts, was not equipped with a guard to prevent contact with rotating parts.
