OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to an ejection hazard: a) In front of the building: On or about June 01, 2018, the forklift operator was exposed to an ejection hazard while operating a JLG Skytrak Telehandler, Model 6036, while not wearing the required seatbelt. b) In front of the building: On or about June 01, 2018, the forklift operator exposed employees to an ejection hazard while operating a JLG Skytrak Telehandler, Model 6036, to elevate two employees on wooden pallets to a height of approximately 15 feet.
