29 CFR 1910.29(b)(1): The top edge height of top rails, or equivalent guardrail system members, were not 42 inches, plus or minus 3 inches, above the walking-working surface: Building 1, Upper Storage Landing: On or about May 30, 2018 and at times prior thereto, the steel top rail was measured at 33 inches above the walking and working surface. This condition exposed employees to a fall hazard of 11 feet 6 inches to the concrete surface below.

This condition exposed employees to inhalation hazards in the event of inadequate supplied breathing air.
