29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:   a.)    On or about May 31, 2018, the employer did not ensure maintenance employees were protected while pulling a 15 amp 32V fuse on the P3 paint line conveyor. Employees were required to replace the fuse and did not wear the required hand and body protection while verifying zero energy, exposing them to electrical shock and arc flash hazards.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:   a.)    On or about May 31, 2018, the employer did not ensure that employees were adequately trained in the lockout procedures for the paint machines, including but not limited to the P8 strip machine.  Employees did not lock out and tag out the electrical disconnect while cleaning the paint from the holes and cup holders and are exposed to crush hazards from the smasher bar.  Larose Industries, LLC dba Cra-Z-Art was previously cited for a violation of this occupational safety and health standard  1910.147(c)(7)(i) which was contained in OSHA inspection number 1210228 , citation number 1, item number 2, and was affirmed as a final order on April 28, 2017, with respect to a workplace located at 250 E. Industrial Pkwy Fayette, OH 43521.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):   a.)    On or about May 31, 2018, the employer did not ensure the P8 strip machine in the Paint Department was properly de-energized by using the established procedures. Employees were exposed to crush hazards from the smasher while wiping out the cup holders and holes with rags and pinch points from the chain and sprockets while changing out the wash bucket while relying on the e-stop for energy control.  Larose Industries, LLC dba Cra-Z-Art was previously cited for a violation of an equivalent occupational safety and health standard, 1910.147(d), which was contained in OSHA inspection number 1210228, citation number 1, item number 1b and was affirmed as a final order on April 28, 2017 with respect to a workplace located at 250 East Industrial Pkwy Fayette, OH 43521.
