29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:  Bay One, on or about 31 May 2018 and times prior thereto; two compressed gas cylinders, one oxygen and one acetylene, that were not in use, were not secured to prevent damage in accordance as required by Compressed Gas Association Pamphlet P-1-1965 . This condition exposed employees to injury from fire and explosion hazard.

Shop Bays Two and Three; on or about 31 May 2018 and times prior thereto; two instances were noted where mechanical machines did not have barrier guards installed.        a. Bay Two, a Masteel Shear; model 12250, serial number 0158, did not have a barrier guard installed to prevent access behind the machine during operations. Miscellaneous items were stored behind the machine.             b. Bay Three, a Masteel Shear; model 12250, serial number 0244, did not have a barrier guard installed to prevent access behind the machine during operations. Miscellaneous items were stored behind the machine.    This condition exposed employees to a struck by hazard.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket).  Shop bays, on or about 31 May 2018 and times prior thereto, three instances were noted where flexible cord sets were not visually inspected for damage:       a. Bay One, a orange 50-foot medium duty extension cord had damage to the outer protective jacket.            b. Bay One, a yellow 50-foot heavy duty extension cord had the outer protective insulation separated from the receptacle plug.       c. Bay Three, a orange medium duty extension cord had the outer protective insulation separated from the receptacle plug.  This condition exposed employees to an electrical hazard.
