29 CFR 1910.28(c)(1): Employer did not protect employees from falling objects by erecting toeboards, screens, or guardrail systems to prevent objects from falling to a lower level: a. 5th Floor - On or about June 5, 2018, a floor hole used as a pipe chase near the vacuum pumps was not protected to prevent objects from falling on to employees using the walkway below. b. 5th Floor - On or about June 5, 2018, a floor edge on the vacuum pump side of the stairway was not protected to prevent objects from falling on to employees using the stairway below.
