29 CFR 1910.106(e)(6)(i): Adequate precautions were not taken to prevent the ignition of flammable vapors from sources such as open flames; lightning; smoking; cutting and welding; hot surfaces; frictional heat; static, electrical, and mechanical sparks; spontaneous ignition, including heat-producing chemical reactions; and radiant heat:   A.  On or about March 22, 2018, Technician Shop:  Precautions were not taken to prevent the ignition of vapors from flammable liquids used in the area of the EnviroWaste parts cleaner equipment.  The Kelsol solvent in the parts cleaner contained Naphtha.  Other flammables used in the area included spray paints and penetrating catalyst and Task Force cleaner, which contained hexane and methanol. Employees were allowed to use open flames in the area. No warning signs were posted in the area.

On or about June 7, 2018, Machine Shop:  Unused portions of the blade on the Wills metal cutting band saw, Model 8M, Serial Number 12643, were unguarded in the following locations:  1.  A 13 1/2-inch section to the left of the point of operation.  2.  An 8-inch section to the right of the point of operation.

On or about June7, 2018, facility-wide:  Employees worked with hazardous chemicals including, but not limited to solvents, brake cleaners and penetrating catalysts.   Container labels indicated flammability as well as eye and skin irritation hazards.  Updated safety data sheets were not available for employee review.

On or about June 7, 2018, Parts Desk:  Access to the 120V General Electric load center was blocked by the placement of a HP 1300 Inkjet printer.  The panel contained breakers which controlled lights, furnace and computers.
