29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Splitter Area: On June 4, 2018 and at times prior, employees were exposed to amputation hazards while operating a Mini Splitter Machine with no guarding of the moving parts of the machine in place. Glacier Stone Supply, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1234102, citation number 1, item number 1 and was affirmed as a final order on 10/18/2017, with respect to a workplace located at 955 Whitefish Stage Rd Kalispell, MT 59901.
