29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about June 5, 2018, at a worksite located at 38601 Mentor Ave, Willoughby, OH 44094, during commercial roofing activities employees were exposed to fall hazards of approximately 25 feet. The employer failed to ensure fall protection systems/devices were used by all employees working on the roof.
