(A) On or about October 10, 2020, at located at Lot 1 on Summer Wind Drive of Four Seasons Residential off Highland Road, Brecksville, Ohio 44141, the employer did not ensure that employees operating pneumatic nail guns used appropriate eye protection to protect employees from flying particles.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about October 22, 2020, employees engaged in roofing activities on 9:12 low-slope roofs, located at Lot 1 on Summer Wind Drive of Four Seasons Residential off Highland Road, Brecksville, Ohio 44141, were not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Top Notch Roofing, Siding, and Additions, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1485981, citation number 1, item number 1, and was affirmed as a final order on October 9, 2020, with respect to a workplace located at 4710 Landchester Cleveland, OH 44109. Top Notch Roofing, Siding, and Additions, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number , citation number 1, item number 2a and was affirmed as a final order on August 6, 2018, with respect to a workplace located at 38601 Mentor Ave, Willoughby, OH 44094.
