29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) Machinery throughout the facility; On or about June 5, 2018 and at times prior thereto the employer exposed employees to potential hazardous energies in that employees were required to perform maintenance and servicing on equipment including but not limited to the Maxson Sheeter Dual Roll Stands Machine, Glue Folder Machine, and the Komori Lithrone G-40 Printing Press without having a hazardous energy control program, without being trained in hazardous energy control, and without machine specific hazardous energy control procedures for those machines with multiple energy sources.

29 CFR 1910.157(g)(1): An educational program was not provided for all employees to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting: a) Facility; On or about June 5, 2018 and at times prior thereto the employer exposed employees to fire hazards in that employees were provided with fire extinguishers for use for fighting incipient stage fires without first being provided training on the principles and hazards involved with fighting incipient stage fires.
