29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):    a) Komori Lithrone G-40 Printing press: On March 13, 2019, and at times prior, the employer exposed employees to potential hazardous energy sources such as, but not limited to electrical, pneumatic, and kinetic, in that the machine specific procedures, did not include multiple steps for locking out the machine and brining it back up.  Specifically, the procedures did not start with notifying affected employees, did not indicate the strength of the energy (ie: Volts, Amps, etc), and did not instruct to Verify Isolation.

29 CFR 1910.106(e)(9)(iii): Combustible waste materials and residues were not stored in covered metal receptacles and disposed of daily:    a) Production area near the Komorie Lithrone G-40 Press: On March 13, 2019 and at times prior, the employer exposed employees to fire hazards in that rags containing flammable residues and vapors were disposed of in uncovered receptacles designated for garbage disposal.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment had been performed through a written certification that identified the workplace evaluated; the person which certified that the evaluation had been performed; the date(s) of the hazard assessment; and, which identified the document as a certification of hazard assessment.    a) Production Area; on March 13, 2018 and times prior, the employer failed to document that a workplace assessment had been performed to determine the hazards, and the necessary personal protective equipment required for the task.
