29 CFR 1926.501(b)(12): Each employee engaged in the erection of precast concrete members (including, but not limited to the erection of wall panels, columns, beams, and floor and roof "tees") and related operations such as grouting of precast concrete members, who is 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems, unless another provision in paragraph (b) of this section provides for an alternative fall protection measure:      a) Job site, 4750 Clairton Boulevard, Pittsburgh, PA: On or about June 7, 2018, during precast concrete grouting activities, the employer did not ensure that appropriate fall protection systems/devices were in use along the edge of the deck panels, exposing employees to fall hazards of up to approximately 17 feet.    Patterson Construction Co., Inc. was previously cited for a violation of this occupational safety and health standard or its' equivalent standard, 29 CFR 1926.501(b)(12), which was contained in OSHA inspection number 935683, citation number 2,  item number 1, and was affirmed as a final order on October 22, 2013, with respect to a workplace located at 524 North Homewood Avenue, Pittsburgh, PA.
