29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  Pinnacle Cove Apartments, 13621 Bayview Isle Drive Bldg #15 - Employees in the process of roofing activities on the three story, 5 in 12 slope (vertical to horizontal) roof, were exposed to an approximate 30 foot fall hazard, in that, fall protection equipment was not installed or utilized, observed on or about 6/8/2018.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a)  Pinnacle Cove Apartments, 13621 Bayview Isle Drive Bldg #15 - On or about June 8, 2018 and times prior to, employees were not instructed as to the hazards associated with their work duties including but not limited to, fall protection requirements when working on steep slope roofs, thus exposing employees to fall hazards up to 30 feet.
