29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): a) On or about 6/12/2018, Employees were doing maintenance/setup on a Multi Saw without having locked out the disconnect which supplies power to the machine leaving the employees with the potential for exposure to unexpected energizing of the equipment.

29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment. On or about 11/28/2018 the employer had not evaluated all areas of the employee's work zone in and around the Heartland Bandsaw Dismantler #1 to maintain a safe distance from the equipment while in use.
