When this is not practical, grab rails which will provide a secure grip for an employee moving to or from the point of access shall be installed.   Barge Eli M., deck:  Portable extension ladders leading from the catwalk below deck to the deck level did not extend 36 inches as required. Employees were exposed to fall hazards.
