29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):    (a) On or about June 11, 2018, Hammersmith Contracting, Inc., as the controlling employer, did not maintain an effective program which provided for frequent and regular inspections of the job site, materials and equipment by a competent person. Employees of White Roofing, Inc. and G&G Roofing, LLC, subcontractors, were exposed to fall hazards of up to 36 feet as follows:        1-Workers engaged in re-roofing activities on a three-story residential building with no fall protection in place.      2-Workers were hoisted to the roof on a Snorkel TB-60 aerial lift without a restraining system to attach themselves to the basket.      3- Four workers weighing a total of 606 Lbs. were hoisted to the roof on a Snorkel TB-60 aerial lift basket that had a maximum capacity of two people and 500 Lbs.                  Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Certification of Corrective Action Worksheet").    Hammersmith Contracting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1048191, citation number 1, item number 1 and was affirmed as a final order on May 19, 2015, with respect to a workplace located at 3233 Zeno Ct. Aurora, CO 80013.    Hammersmith Contracting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1192400, citation number 1, item number 1 and was affirmed as a final order on December 30, 2016 with respect to a workplace located at 218-220 Whitehaven Circle, Highlands Ranch, CO 80129.
