29 CFR 1910.147(d)(4)(i): Lockout or tag out devices were not affixed to each energy isolating device by authorized employees: (a) This violation occurred on or about June 06, 2018, when an employee working in the production department and unjamming the Jumbo Palletizer Machine, model number, P7500-MKII-4840-RH-REPD-RAA, serial number 2442, was exposed to a caught-in-between hazard because the employer failed to fully document the training of the employee in the procedure to be utilized for unjamming the Jumbo Palletizer Machine model number, P7500-MKII-4840-RH-REPD- RAA, serial number 2442.
