On or about June 12, 2018, during the demolition of a school building, located at 1025 Lake Center Street., NW, Uniontown, Ohio, the employer did not ensure employees working from the roof (walking/working surface) were protected from a fall of approximately 10 feet, 8 inches, by utilizing a guardrail system, safety net system and/or personal fall arrest system.
