29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b)(13). On or about June 15, 2018, the employer did not ensure employees were in use of fall protection while working from a roof installing asphalt shingles, exposing the employees to a fall hazard of more than six (6) feet to the ground below. The employer MS Eagles Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501 (b)(13), which was contained in OSHA inspection number 1119150, citation number 001, item number 01. and was affirmed as a final order on May 20,2016, with respect to a workplace located at 509 NW 179th St. Edmond, OK 73012. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about June 15, 2018 and at times prior thereto employees were exposed to fall hazards while working installing asphalt shingles on new construction residential home by employees improper set up and use of extension ladder to access roof. The employer visits site and failed to reinforce requirement to have a minimum of three feet extended above landing for safe access. The employer MS Eagles Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053 (b)(01), which was contained in OSHA inspection number 1119150, citation number 002, item number 02. and was affirmed as a final order on May 20, 2016, with respect to a workplace located at 509 NW 179th St. Edmond, OK 73012.
