29 CFR 1926.20(b)(4): The employer did not ensure the employee(s) required to operate equipment and machinery was qualified by training or experience. On or about June 5, 2018, the employer allowed an employee who was not qualified by training or experience to operate a vibratory roller at the commercial construction worksite, and who was not wearing a seatbelt while operating the equipment. This exposed the employee to roll-over hazards.
