29 CFR  1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c):  On or about September 8, 2021, at an excavation site located in a field on MacArthur, south of NW 150th in Oklahoma City, OK, no protective devices were in place to protect employees working in a trench that was more than 5 feet in depth.  The employees were exposed to struck-by hazards.
