29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a)  6276 Hinsdale Ave, Centennial CO:  On or about May 15, 2018, and at times prior, Hoffman Weber Construction, as a controlling employer, did not ensure that SN Construction protected their employees from being exposed to fall hazards due to working on first story roofs without adequate fall protection measures.  One employee was installing a gutter after siding repair had been completed.  The employee was standing on first story roofs lacking fall protection measures such as personal fall arrest systems, safety nets, or guardrails.    (b)  6296 Hinsdale Ave, Centennial CO:  On or about May 15, 2018, and at times prior, Hoffman Weber Construction, as a controlling employer, did not ensure that SN Construction protected their employees from being exposed to fall hazards due to working on first story roofs without adequate fall protection measures.  One employee was cleaning debris off another house after tear-off of siding.  The employee was standing on first story roofs lacking fall protection measures such as personal fall arrest systems, safety nets, or guardrails.    (c)  6050 East Hinsdale Court, Centennial CO:  On or about May 15, 2018, and at times prior, Hoffman Weber Construction, as a controlling employer, did not ensure that SN Construction protected their employees from being exposed to fall hazards due to working on second story roofs without adequate fall protection measures.  Two employees were installing siding on a two story house.  There was only one lifeline provided to employees, however both employees were wearing harnesses.  Personal fall arrest systems in place were inadequate to protect both employees at the same time. 29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed    (a) 6296 Hinsdale Ave, Centennial CO: On or about May 15, 2018, and at times prior, Hoffman Weber Construction, as a controlling employer, did not ensure that SN Construction protected their employees from being exposed to fall hazards due to stepladders being used in positions for which they were not designed.  The stepladders noted during inspection were designed for use on level surfaces with the spreader bars in an open and locked position.  Employees were using two stepladders with closed spreader bars on angled surfaces while working on first story roofs.      (b) 6050 East Hinsdale Court, Centennial CO:  On or about May 15, 2018, and at times prior, Hoffman Weber Construction, as a controlling employer, did not ensure that SN Construction protected their employees from being exposed to fall hazards due to stepladders being used in positions for which they were not designed.  The stepladders noted during inspection were designed for use on level surfaces with the spreader bars in an open and locked position.  Employees were using one stepladder with a closed spreader bar on an angled surfaces while working on first story roofs. Employees utilized three unsecured ladders on roofs with a slope of approximately 56 degrees.  (b) 6050 East Hinsdale Court, Centennial CO: On or about May 15, 2018, and at times prior, Hoffman Weber Construction, as a controlling employer, did not ensure that SN Construction protected their employees from being exposed to fall hazards due to using unsecured ladders on surfaces that were not level.  Employees utilized one unsecured ladder on a roof with a slope of approximately 56 degrees. 29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)    (a) 6276 Hinsdale Ave, 6296 Hinsdale Ave, 6050 E. Hinsdale Court, Centennial CO 80112: On or about May 15, 2018, and at times prior, Hoffman Weber Construction Inc, as a controlling employer, did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials, and equipment in use at the job site.  Daily inspections of the worksite conducted by the employer were not effective in enforcing safety regulations.    Abatement Note: Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).    Hoffman Weber Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.20(b)(2), which was contained in OSHA inspection number 1006245, citation number 1, item number 1and was affirmed as a final order on 12/22/2014, with respect to a workplace located at24694 Quarto Place, Aurora CO 80016.
