29 CFR 1910.217(e)(1)(i)(A): Periodic and regular inspections of each power press were not conducted to ensure that all of its parts, auxiliary equipment, and safeguards, including clutch/brake mechanism, anti-repeat feature, and single-stroke mechanism, are in safe operating condition and adjustment:      On or about June 15, 2018, and at times prior thereto and thereafter, in the small order department, employees used punch presses to manufacture small metal parts. The employer did not include inspecting safety mechanisms in clutch/brake assemblies, such as but not limited to, the adjusting nut lock, in their periodic inspections.    PER EXECUTED ISA AVD IS MODIFIED TO:    On or about June 15, 2018, and at times prior thereto and thereafter, in the small order department, inspections performed on the power press machinery were inadequate; in that the inspections did not include the press brake/clutch system to include its safety features and components.
