29 CFR 1910.184(i)(9): Synthetic web sling was not immediately removed from service for having snags, cuts and tears.    On or about March 12, 2020, a synthetic web sling used in the punch press tool room by an employee to lift and/or move dies that weighed approximately 75(+) lbs. was defective in that the stitching was worn, and there was visible fraying and snags throughout the sling. The defective sling was not removed from service.

1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.      On or about 12/03/2019, the employer did not ensure that various equipment/ machinery used at the site was effectively guarded.  The unguarded equipment includes; but is not limited to, the following:     a. The four-slide and multi-slide machinery located in the Slide Forming Department was inadequately guarded. Although presence sensing guarding was in place, the system did not prevent employees from being able to reach under the guarding, potentially exposing the operator rotating parts;     b. Bridgeport milling machines located in entry of the punch press tooling area were unguarded and thus exposed the operator to struck by hazards from the fly pieces of scrap metal.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:       On or about 12/03/2019, adequate strain relief was not provided for the set screw/cord grip of a machine lamp on the #27 four slide machinery. The set screw/cord grip used for routing the electrical wires into the lamp was not attached, and thus potentially exposed workers who frequent the area to electrical hazards should the wiring be damaged and/or ripped out.
