29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a. On or about June 13, 2018, in the production room, employees were exposed to chemicals including, but not limited to ethyl 2-cyanoacrylate while splash loading hazardous materials. The employer failed to provide and require the use of appropriate body protection when transferring hazardous materials.

29 CFR 1910.1200(h)(3)(iii): Employee training did not include the measures employees can take to protect themselves from chemical hazards, including specific procedures the employer had implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures and personal protective equipment to be used: a. On or about June 13, 2018, in the production room, an employee was exposed to ethyl 2-cyanoacrylate during a spill while transferring the chemical. The employer failed to train employees on proper emergency procedures to be taken in the event of significant skin contact with chemicals including, but not limited to ethyl 2-cyanoacrylate.
