This exposed employees to a fall hazard of approximately 9.6 feet

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person. (a) The Northern most anchorage point was not installed correctly exposing employees to a fall hazard on a 6/12 pitch roof with a 9.6 foot fall. (b) The center most anchorage point was not installed correctly exposing employees to a fall hazard on a 6/12 pitch roof with a 9.6 foot fall. (c) The Southern most anchorage point was not installed correctly exposing employees to a fall hazard on a 6/12 pitch roof with a 9.6 foot fall.
