29 CFR 1926.252(a): Whenever materials are dropped more than 20 feet to any point lying outside the exterior walls of the building, an enclosed chute of wood, or equivalent material, shall be used. For the purpose of this paragraph, an enclosed chute is a slide, closed in on all sides, through which material is moved from a high place to a lower one. a. On or about April 25, 2022, at the above addressed jobsite, an employee performing roof tear off operations was dropping material approximately twenty-five (25) feet to the ground with no chute in place, thereby exposing employee(s) to struck-by hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a. On or about April 19, 2022, at the above addressed jobsite, employees performing residential roofing operations, nineteen (19) feet two (2) inches above the ground, was not protected from falls, thereby exposing the employees to fall hazards. Top Property Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1564826, citation number 1, item number 1 and was affirmed as a final order on February 2, 2022, with respect to a workplace located at 214 S. Fairfield Ave., Elmhurst, IL. 60126. Top Property Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1532196, citation number 1, item number 1 and was affirmed as a final order on October 18, 2021, with respect to a workplace located at 1614-1616 Fieldstone Dr. Shorewood, IL 60404.
