29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. a) Allan J Construction LLC located at 4235 S. College Ave., Fort Collins, CO 80525: On and preceding 6/20/2018, the employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles. Six employees were installing shingles on a commercial roof using pneumatic nail guns without the use of eye protection. This condition exposed the employees to struck by injuries to the eyes.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) Allan J Construction LLC located at 4235 S. College Ave., Fort Collins, CO 80525: On and preceding 6/20/2018, the employer did not ensure that each employee on a commercial roof with an unprotected side or edge which was 6 feet or more above a lower level was protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. A crew of 6 roofers was singling a 3/12 pitch roof without utilizing fall protection. Two roofers were wearing harnesses, but were not connected to a lanyard. The remaining roofers were connected to the front D-ring of their harnesses instead of the back D-ring, and their chest and leg straps were not connected. This condition exposed employees to a fall hazard of approximately 10 feet.

A crew of 6 roofers was singling a 3/12 pitch roof without utilizing fall protection. Several roofers were connected on the front D-ring of their harnesses instead of the back D-ring. This condition exposed employees to a fall hazard of approximately 10 feet.

A crew of 6 roofers was singling a 3/12 pitch roof without utilizing fall protection. Several roofers were not wearing their harnesses properly in that they were connected to the front D-ring of their harnesses in stead of the back D-ring, and their chest and leg straps were not connected. This condition exposed employees to a fall hazard of approximately 10 feet.
