29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects:  (a) Allan J. Construction, LLC., at 7043 Montana Place, Denver, CO 80224: On or before June 15, 2020, the exposing employer did not ensure that employees wore protective helmets during re-roofing activities.  Employees were working on the ground and first level of a split-level residential house while other employees were working above on the second level, preforming re-roofing activities.  This condition exposed employees to head injuries from falling objects.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a) Allan J. Construction, LLC., at 7043 Montana Place, Denver, CO 80224: On or before June 15, 2020, the employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  Four employees were installing shingles on a residential roof using pneumatic nail guns without the use of eye protection.  This condition exposed employees to struck-by hazards to the eyes.  Allan J. Construction, LLC., previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1324363, citation number 1, item number 1 and was affirmed as a final order on June 20, 2018, with respect to a workplace located at 4235 S. College Ave, Fort Collins, CO 80525.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501   (a) Allan J. Construction, LLC., at 7043 Montana Place, Denver, CO 80224: On or before June 15, 2020, the exposing employer did not ensure their employees were protected from injury, in that employees actively engaged in roofing activities on top of a residential home were not utilizing a fall protection system.  This condition exposed employees to a fall hazard of 10-20 feet.  Allan J. Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13); which was contained in OSHA inspection number 1191563, citation number 1, item number 1  and was affirmed as a final order on January 3, 2017, with respect to a workplace located at4003 Quince Street, Denver, CO 80237.
