29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:  (a) Employees opening and closing electrical circuits feeding dry type 480 Volt electrical transformers that were being tested while energized were exposed to arc-flash/arc-blast hazards in that they stood at close proximity from the transformers, such as but not limited to transformer B4AL in room 4B-416 installed on or about 07/11/18.
