29 CFR 1926.1053(b)(10): The tops of a non-self-supporting ladder were not placed with the two rails supported equally: a) 455 Stanton-Christiana Road, Newark, DE - On or about May 30, 2018, the employer failed to ensure that the two rails of a 24' Werner extension Ladder were equally supported on a utility pole approximately 34-inch in diameter. Employees using the ladder were exposed to fall hazards from heights approximately 18 feet above grade.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards. a) 455 Stanton - Christiana Road, Newark, DE - On or about May 30, 2018, the employer failed to provide training, related to the hazards and proper use of ladders, to employees using a 24-foot Werner extension ladder to disconnect cables on a utility pole. Abatement Note: The employer shall ensure that each employee has been trained by a competent person in the following areas, as applicable: -The nature of fall hazards in the work area; -The correct procedures for erecting, maintaining, and disassembling the fall protection systems to be used; -The proper construction, use, placement, and care in handling of all stairways and ladders; -The maximum intended load-carrying capacities of ladders and; -The standards contained in this subpart. Retraining shall be provided for each employee as necessary so that the employee maintains the understanding and knowledge acquired through compliance with this section.
