29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.   On or about 21 June 2018, and at times prior thereto, six roofers were performing roofing activities at the ground level while removal and installation of roofing materials were underway above on the 2nd story of two separate apartment units without the use of head protection, exposing the roofers to the hazard of being struck by falling tools, materials, or debris.

29 CFR 1926.102(a)(1): 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.    On or about 21 June 2018, and at times prior thereto, a roofer was using a pneumatic nail gun to apply shingles to a 2nd story apartment building roof without the use of safety glasses, exposing the roofer and those nearby to the hazard of being struck by flying particles and debris.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On or about 21 June 2018, and at times prior thereto, eight roofers were performing roofing activities on the 2nd story of two separate apartment buildings without the use of fall protection, exposing the roofers to a fall hazard of approximately 18 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.   On or about 21 June 2018, and at times prior thereto, four roofers accessed the 2nd story roof of an apartment building without the ladder being extended at least 3 feet above the upper roof landing, exposing the roofers to a fall hazard of approximately 18 feet to the ground below.
