29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: On or about June 18, 2018, the employer did not ensure the established procedure for the application of energy control (lockout/tagout) and was performed in sequence during the setup and adjustment of the No. 45 large Automatic Roll Machine. Employees did not apply energy controls by; turning off and/or shutting down the press [per the 1910.147(d)(2) requirements], isolating all energy sources [(d)(3)], affixing a lockout device to each energy isolating device [(d)(4)], and verify the equipment energy was isolated and de-energized prior to starting work [(d)(6)].

29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed. On or about June 18, 2018, the employer did not ensure periodic inspections of energy control procedures were being performed at least annually to ensure the requirements of the standard were being followed. The employer did not ensure an inspection was conducted of the No. 45 large Automatic Thread Roll Machine energy control procedure to correct any inadequacies or deviations in the procedure [per 1910.147(c)(6)(i)(B) requirements], and/or annual periodic inspections included a review of each authorized employee and their responsibilities under the energy control procedure [(c)(6)(i)(C)].
