29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.     On or about June 26, 2018, during the installation of roofing material at the job site located at 36409 Capri Ln, North Ridgeville, Ohio 44039; the employer did not ensure employees working on the roof of a new construction single family home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:     On or about June 26, 2018, during the installation of roofing material at a job site located at 36409 Capri Ln, North Ridgeville, Ohio 44039; the employer did not ensure employees were properly utilizing personal fall protection systems. The employees were engaged in the installation of roofing material on a residential roof with a pitch of 6/12, each employee had on a PFAS, the employees were utilizing vertical life lines that were not adjusted to limit a free fall to less than 6 feet. The roof height was approximately 12.3 feet above the ground.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:     On or about June 26, 2018, during the installation of roofing material at a job site located at 36409 Capri Ln, North Ridgeville, Ohio 44039 the employer did not ensure employees working on a 6/12 and 10/12 pitched roof were protected from a fall of approximately 12.3 feet by utilizing a fall protection system correctly and by providing fall protection training or fall hazard recognition.
