29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: On or about October 26, 2018, during the installation of roofing material on a newly constructed residential home located at 3801 Lake Rd. in Sheffield, Ohio, the employer failed to ensure that employees were protected from a fall hazard by conventional fall protection and/or devices. Employees were exposed to fall hazards from heights of approximately 13 - 17 (+) feet, from the top of the roof to ground level.

29 CFR 1926.102(a)(1): Eye and/or face protective equipment was not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: On or about October 26, 2018, during the installation of roofing material at the job site located at 3801 Lake Rd. in Sheffield, OH, 44054, the employer failed to ensure that employees working on the roof of a newly constructed home were protected from possible eye injuries from impact or flying objects from other employees installing roofing material. Two employees were using pneumatic nail guns to secure roofing material to structure without using safety eye-wear or facial protection. Lagorga Construction Llc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.201 (a) (1), which was contained in OSHA inspection number 1325202, citation number 1, item number 01 and was affirmed as a final order on August 06, 2018, with respect to a workplace located at 36409 Capri Ln., North Ridgeville, OH 44039.
