You must also consider a case to meet the general recording criteria if it involves a significant injury or illness diagnosed by a physician or other licensed health care professional, even if it does not result in death, days away from work, restricted work or job transfer, medical treatment beyond first aid, or loss of consciousness. East Chicago, Indiana terminal: on or about June 13, 2018, the employer failed to consider a truck driver's work-related death as a recordable and reportable injury, and did not record it in the OSHA 300 Log within seven calendar days from occurrence. There is no abatement certification or documentation required for this item.
