29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6):    On or about June 26, 2018, the employer failed to ensure that the scaffold erected in place and used by employees was kept the minimal distance from the infeed power line to the home as indicated in table in paragraph (f)(6) of this section. The ladder jack scaffold in use was identified as being only 24 inches from the uninsulated infeed line.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    On or about June 26, 2018, the employer failed to ensure that employees working from a ladder jack scaffold system were protected by the use of a personal fall arrest system (PFAS). Employees were working from the scaffold which was approximately 17 feet from the ground below.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about June 26, 2018, the employer failed to ensure that employees were protected from falling from the roof while performing tear off activities. Personal fall arrest systems were available on-site, however employees were observed not utilizing them. The roof had a pitch of 11/12, and the fall height was approximately 20 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about June 26, 2018, the employer failed to ensure that extension ladders used on site to gain access to the roof were extended at least 3 feet above the roof line. Four ladders were used to gain access to the roof and were not extended at least three feet.

29 CFR 1926.404(b)(1)(iii)(C): The employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage;  On or about June 26, 2018, the employer failed to visually inspect cords being used onsite prior to their use. An extension cord was being used to supply power to a handheld saw at the time of inspection. The cord contained visible damage.
