29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  The employer failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1 Item 1 for eye/face hazards and lack of eye protection and Citation 2 Items 1a/b for fall hazards and lack of fall protection systems/devices and training.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   On or about October 1, 2019, the employer did not ensure that eye protection was worn by employees utilizing air powered nailers to install shingles on the roof of a residential home located at 6840 Hunting Lane, Independence, OH, 44131.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  On or about October 1, 2019, the employer did not ensure that fall protection was worn by employees utilizing a ladder jack scaffold that was 16.6 feet above ground level to install shingles on the roof of a residential home located at 6840 Hunting Lane, Independence, OH, 44131.  JR Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.451(g)(1)(i),  which was contained in OSHA inspection number 1325342, citation number 1, item number 2a, and was affirmed as a final order on August 7, 2018, with respect to a workplace located at 696 Avon Belden Road, Avon Lake, OH 44012.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about October 1, 2019, the employer did not ensure that fall protection was worn by employees utilizing stripping shingles and installing shingles on the roof of a residential home located at 6840 Hunting Lane, Independence, OH, 44131. Employees were exposed to a fall of 19 feet to the ground below.   JR Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(13), which was contained in OSHA inspection number 1325342, citation number 1, item number 2b, and was affirmed as a final order on August 7, 2018, with respect to a workplace located at 696 Avon Belden Road, Avon Lake, OH 44012.
