29 CFR 1910.22(a)(2):  The floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition.  When wet processes are used, drainage must be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats must be provided:    On or about June 27, 2018, at a facility located at 4243 Gateway Drive, Sheboygan, WI 53081; the employer did not maintain the floor of each workroom in a clean and in a dry condition. The floors of the 3500 Ton and 1600 Ton departments were slick due to residue from the die casting process. Forklifts were observed spinning tires attempting to get traction with the floor. On or about July 1, 2018, a forklift lost control and crashed into a die cast machine because of the slick floor.

29 CFR 1910.1200(f)(6)(ii):  The employer shall ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked with product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available to employees under the hazard communication program, will provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:    On or about June 19, 2018 at a worksite located at 4243 Gateway Drive, Sheboygan, WI 53081; the employer did not ensure plastic totes containing ceiling residue scrapings and liquid from floor washing of residue; which contained lead, cadmium and inorganic arsenic; were labeled, tagged or marked with information to identify the contents or the health hazards of the chemicals within.
