29 CFR 1910.22(a)(2): The floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition. When wet processes are used, drainage must be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats must be provided: On or about June 27, 2018, at a facility located at 4243 Gateway Drive, Sheboygan, WI 53081; the employer did not maintain the floor of each workroom in a clean and in a dry condition. The floors of the 3500 Ton and 1600 Ton departments were slick due to residue from the die casting process. Forklifts were observed spinning tires attempting to get traction with the floor. On or about July 1, 2018, a forklift lost control and crashed into a die cast machine because of the slick floor.

29 CFR 1910.1200(h)(3)(ii): Employee training shall include at least the physical, health, simple asphyxiation, combustible dust, and pyrophoric gas hazards, as well as hazards not otherwise classified, of the chemicals in the work area: On or about June 19, 2018 at a worksite located at 4243 Gateway Drive, Sheboygan, WI 53081; the employer did not provide training to employees on the health hazards of lead, cadmium and inorganic arsenic. Employees were working in areas where residue from die casting process was present on equipment and surfaces. The residue contained lead, cadmium and inorganic arsenic.
