29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: On or before, June 28, 2018, employees were exposed to fall hazards of approximately 10.9 feet while performing load-out operations on the load out platform on SFI building 772 and the employer did not ensure that an appropriate fall protection was utilized.

29 CFR 1910.29(b)(3): Guardrail system was not capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied in a downward or outward direction within 2 inches (5 cm) of the top edge, at any point along the top rail. On or before June 28, 2018, employees were exposed to fall hazards of approximately 10.9 feet to the pavement and the employer did not ensure that the temporary guardrail installed was capable of withstanding, without failure, force of at least 200 pounds (890 N) applied in a downward or outward direction.
