29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate face protection when exposed to face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. Location: Gas House, Tank Farm, LVM and HVM Mix Rooms, Reactor House, and other areas where chemicals are sprayed, and where they are pumped or poured into open containers, barrels, or drums. Employees were exposed to hazardous chemical splashes during pumping and pouring of chemicals into containers, totes and drums, repairing equipment containing hazardous chemicals, packaging of adhesives, and filling and testing of cylinders inside and outside the gas house.
