29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) Job site, 4750 Clairton Boulevard, Pittsburgh, PA: On or about June 26, 2018, during roofing activities, the employer did not ensure that appropriate fall protection systems/devices were in use around the roof hatch opening in that caution tape was placed around it, in some places as close as approximately 6 inches from the hatch, exposing employees to fall hazards of up to approximately 11 feet.

29 CFR 1926.503(c): When the employer had reason to believe that any affected employee who had already been trained did not have the understanding and skill required by paragraph (a) of this section, the employer did not retrain each such employee: a) Job site, 4750 Clairton Boulevard, Pittsburgh, PA: On or about June 26, 2018, during roofing activities, the employer did not ensure that appropriate fall protection systems/devices retraining was conducted where appropriate. Deficiencies included, but were not limited to flag heights being too low or excessive rope amounts being used near the edge of the roof on rope grab systems.
