29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   a) Building Exterior/Front: Employees working without hard hat were exposed to a struck by hazard from roofing shingles, leaf blower, workers on the roof and tools, on or about 6/27/2018.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   a) Roof: Employees engaged in roofing work were exposed to a fall hazard of approximately 30 feet while working without a personal fall arrest system or other means of fall protection on a roof with a pitch of 6/12. On 06/27/2018 an employee fell approximately 30 feet while working from a roof without a fall protection system in place.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a) Building Exterior/Front: Employees were using a 30 foot Werner extension ladder that did not extend 3 feet above the landing to access a roof, on or about 6/27/2018.
